Postdural puncture headache is one of the complications of central blockades.  It arises most often in connection with a previous puncture of  epidural or spinal space as a result of perforation of the dura mater and arachnoidea. A cerebrospinal fluid is leaking from the gap in dura mater. It leads to a drop  of pressure of cerebrospinal fluid and it can cause headaches. The patient is usually warned by obvious symptoms, but sometimes the eclampsia appears unexpected. It requires immediate intervention of seizure suppression, high blood pressure decrease and quick delivery. 76letá pacientka v domácí péči, ošetřována rodinou. Poslední dny nejedla, málo pila. Obtížný kontakt. Na emergency přivezena pro progresi poruchy vědomí a rozvoj febrilí s podezřením na uroinfekt. Rychle progredující ventilační selhání indikuje k napojení na UPV, nutná podpora vazopresory. Nasazena empirická ATB terapie - Amoksiklav 1,2 g i.v. co 8 hod k pokrytí ev. močové infekce. Po rychlé progresi celkového stavu došlo po 8 hodinách pobytu v nemocnici k exitu. Pitevní nález potvrdil jako zdroj sepse nejen močový infekt, ale zejména přítomnost infikovaných chlopenních vegetací s perikarditidou.

RZP přiváží na Krizové centrum FN Brno ženu po intoxikaci medikamenty. Dle dostupné dokumentace je somaticky sledována pro hypothyreózu (užívá Euthyrox), jinak zdravá. Je svobodná, bezdětná, více informací není aktuálně k dispozici. Dle posádky RZP pacientka zanechala dopis na rozloučenou (dopis s sebou nemají) a užila ne zcela známé množství léků (cca 40 tbl. Rivotril 0.5 mg). Na KC přivezena na lehátku, pro výraznou malátnost není schopná přejít z lehátka na sedačku.
